Any rush orders or special shipping requests will need to be arranged and paid for in advance.  If you are ordering a 16x20 or 20x30 size, please call or email first to verify that the original photo will allow this.  Indoor and low light photographs may have a grainy look which may not be recommended for this size.  All special requests will be discussed prior to the order and payment.

We accept all personal checks and money orders made out to Elite 1 Imaging and mailed to the address on the top of this form. Your order will be processed and shipped within 3-5 business days of receipt. Checks that are returned for non-sufficient funds will be surcharged $25.00. Elite Sports Photography reserves the right to refuse service to anyone, for any reason. You agree to all the terms and conditions noted on our website (www.elite1imaging.com) and this form with your order.


If you are making payment by mail, please print this form and attach it with your payment, if you are making payment through Pay Pal, please "Submit" the form for our processing. It is suggested that you print and keep a copy of this order form for your records.
